1.       Evidence of U.S. citizenship with a birth certificate, naturalization or citizenship certificate, or copy of your passport

  1. A copy of your birth certificate that proves the relationship between you and your parents (Parents/Spouse)
  2. If there was a legal name change, you need to provide proof of that (Parents/Spouse)
  3. Two passport photographs

 

Sponsor Side  

a.   A copy of your most recent federal income tax returns along with W-2s

b.   If you have any 1099s, Schedules, or other reported income, include copies of those

c.    Your most recent six months of pay stubs can help you qualify

d.   A signed letter from an employer can help qualify

e.    Include copies of your most recent Schedule C, D, E, or F if you’re self-employed

f.     If you’re using assets to meet the income requirement, you need to provide documentation about your assets

g.   Evidence of U.S. citizenship with a birth certificate, naturalization or citizenship certificate, or copy of your passport

Parent/children/spouse side

a.       Two passport-style photographs for each parent

  1. Copies of:
  2. Any documents relating to any criminal charges, arrests, or convictions for your parents
  3. A full copy of your parents’ medical history
  4. A copy of their vaccination and immunization records
  5. Any copies of previous chest x-rays
  6. A doctor’s letter outlining treatment plans for any medical conditions your parents may have
  7. Health insurance card if their insurance covers the exam
  8. Government-issued photo identification
